CAUSES OF HIP PAIN
Hip pain can arise from various causes, which can be broadly categorised into three groups:
Hip Pathology
- Inflammation of tendons or other soft tissues - tendinitis, tendinosis, bursitis
- Osteoarthritis - wear and tear of the cartilage stuck to the bones (articular cartilage); it can also be associated with meniscus tears (meniscus is the cartilage between the bones, which acts like a shock absorber when walking or doing physical activities)
- Inflammatory arthritis such as rheumatoid arthritis, ankylosing spondylitis, gout and pseudogout, lupus, psoriatic arthritis
- Rare causes including infections, tumours, osteonecrosis (lack of adequate bone supply, for various reasons, leading to dead bone)
- Loosening or infection of a previous hip replacement
Non-Hip Pathology
(Referred Pain)
- Spine osteoarthritis - usually associated with sciatic or femoral nerve type symptoms
- Sacro-iliac joint (joint connecting the spine with the pelvic bone) pathology
- Inguinal or femoral hernia
- Pinched nerve around the front of the hip - meralgia paresthetica
- Pinched nerve around the buttock area (deep gluteal pain syndrome)
Trauma

Diagnosis of the Hip Problem
Clinical Diagnosis
A thorough clinical examination, including gait and posture examination, inspection and palpation of the painful area, range of movement examination and special tests to identify either a fixed muscle contracture around the hip or muscle weakness.
Imagistic Diagnosis
(to help with definitive diagnosis and management)
- X-ray Hips – antero-posterior and lateral view (a calibrating marker can be used in cases where digital templating might be necessary for the treatment).
- CT, bone scan and MRI (+- contrast) – rarely necessary, when X-rays are not enough for a definitive diagnosis.
Diagnostic Injection
Local anaesthetic injection into the hip joint – used rarely, particularly when the pain is not only in the hip area; Mr Arvinte does hip injection only in a theatre – a sterile environment, under Xray guidance.
